JMS Registration Information

The following registration forms are required for the JMS Registration and Open House on July 31st from 3:30-6:30 p.m.  To speed up your visit, you may type, print, and complete these forms at your convenience for your student.  Please remember that additional forms may be needed when you arrive for registration.
