What if I can't afford the polo shirts? 
The requirement is only polo style shirts ~ not shirts made by Ralph Lauren Polo.

Can cardigans have initials on them?
No, they can have a Jefferson Logo on them, but not initials

Do shoes have to be certain color or match dress code?
No

Can my child change clothes for PE?
No, not enough time

In winter can long sleeve t-shirts just be worn under the short sleeve shirt or does it have to be long sleeved? Yes, students can wear long sleeve t-shirts, long sleeve under-armour, etc. under a polo style shirt if they wish OR they can purchase long sleeve "polo style" shirts.  Either one is permissible.
